Yikes! – I Have Spots on the Back of My Hands!

Yes, your hands can get brown spots from over sun exposure just like the rest of your skin. The back of your hands are exposed to a lot of sun light over the years and it is an area we hardly think of protecting with sunscreen. So the result is Lentigos – more commonly called age spots or liver spots – although these spots have nothing to do with age or your liver.

Lentigos on Hand

Lentigos are formed when your skin is trying to protect itself from UV rays. It produces more melanin (skin pigment) to defend itself, but sometimes the melanin accumulates in one area which creates the brown spot. Lentigos are basically harmless, but no one likes them on their hands.

 

The treatment for Lentigos on the hands is the same as for anywhere else. However, keeping enough skin care products on your hands is tough if you tend to wash often during the day. Bedtime is a great time to apply bleaching products to your spots since you will sleep all night before washing the bleaching product off.

So how do I get rid of these ugly spots?

The two key points to erasing Lentigos are:

  • Stopping the formation of new melanin at the spot area – do this by protecting your hands with sun screen.
  • Removing the existing pigmentation causing the spot – this can be done several ways.

The treatment options are-

Over the counter skin care products:

hydroquinone
  • Bleaching (depigmenting) agents which include-
    • Hydroquinone (2% over the counter strength and 4% Rx strength available)
    • Kojic Acid
    • Azelaic Acid
    • Arbutin
    • Ellagic Acid
  • Retinoids (Retinoic Acid)
  • Vitamin C (Ascorbic Acid)
  • Alpha Hydroxy Acids

Other ingredients being studied for skin lightening effects:

  • Bearberry Extract
  • Licorice Extract
  • Mulberry Extract

Treatments available through a professional:

  • Laser Resurfacing (less damaging to skin)
  • Chemical Face Peels (irritating but faster than over the counter products)
  • Freezing the area with liquid nitrogen (may cause white spots)

Bleaching products work by inhibiting the production of the enzyme tyrosinase, which in turn reduces melanin. Over time, the brown spots will fade. Bleaching products will not lighten skin beyond its’ own natural color, so do not worry about going to light.

Retinoids and Alpha Hydroxy Acids can slowly fade brown spots on their own. However, they are best used in conjunction with bleaching products since they will boost the overall effectiveness of the treatment.

A doctor or aesthetician would perform the bottom 3 options.

So if you do not wish to seek professional help, here is what you need to do to fade your brown spots:

Choose a bleaching agent based on your skin's sensitivity and coloring.

  • fair to light/medium skin can use hydroquinone
  • medium/ darker skin should use kojic acid, arbutin, azeliac acid, ellegic acid
  • sensitive skin should use hydroquinone with caution - test skin first

Then look for a product that combines the bleaching agent that is right for your skin color with a retinol and an AHA or BHA. This is the easiest way to get a product that will work for your hands. Having to apply layers of lotions is impractical when it comes to the hands.

What I do is keep my product with me at my desk so I can re-apply a few drops throughout the day while working. My other trick is – I apply a few drops to the back of one hand and use the back of my other hand to rub it in. Then I do not have residue on my palms. I also apply a few drops at bedtime.

With the right product, you should see results in 6 to 8 weeks. Remember to keep using sunscreen or new spots will show up. You can have nice hands again!
